Rotator cuff tears are common and range from small partial tears to massive chronic tears. Treatment is individualized. Many degenerative tears improve with rehabilitation and symptom management. Acute traumatic tears with weakness and repairable full-thickness tears may be better suited to early surgical assessment.

Clinical examination, imaging, functional demands, tear morphology, and patient goals all contribute to decision-making.
